TAX-2642 replaced call to action to email with a link to a form instead

We want to streamline the process to register a tax provider by making potential integrators use a form instead. This should reduce headaches with not getting the required info/info in the wrong format

What changed?

• Replaced call to action to send an email with a call to action to click and submit the form
• Removed the table since the form now describes all the relevant information
